Our Telehealth Community Navigation Contact Center meets the needs of this vulnerable population virtually. 

Whether it’s by phone, text, email, or a social media private message, we build health assessment profiles for each individual and then guide them to resources addressing social determinants of health and/or medical services, with timely follow-ups to ensure care gaps are eliminated. We average 4000 calls a month.

Instructors lead Promotores and Community Health Workers in a dynamic advocacy session, equipping them to amplify community voices in policy and healthcare spaces. Through education and empowerment, they prepare trusted leaders to stand confidently at decision-making tables and champion health access for all.

Promotores and Community Health Workers conduct vision screenings, ensuring early detection and access to eye care for community members who may otherwise go without. Their hands-on guidance and culturally responsive support help families take the first step toward clearer vision and healthier futures.  

A Promotora stands confidently with a microphone, sharing her lived experience and speaking from the heart to a room filled with fellow Community Health Workers and partners. Her story reflects the power of trusted voices in community spaces, where personal testimony serves as a bridge to education, healing, and collective action.
